BrightCare Treatment ModelOverview
BrightCare is a comprehensive, outcomes-driven mental health treatment model grounded in the principles of the Legacy Effect. Developed in collaboration with Deschutes County Mental Health, High Lakes Healthcare Primary Care, Mosaic Medical Primary Care, PacificSource CCO, the Health Council in Bend, and Creach Consulting, BrightCare emphasizes early intervention, rapid access, robust services, coordinated care, and continuous measurement.
The Legacy Effect demonstrates that early, intensive intervention leads to better outcomes and lower long-term costs. This approach has been successful in chronic disease management and is especially impactful in mental health, where over 75% of emergency department overutilization involves individuals with mental health conditions.
Core Principles of the BrightCare Model
1. Rapid Access
BrightCare prioritizes immediate availability because early access significantly increases engagement.
• Engagement increased from 26% to 75% in studies with High Lakes Healthcare.
• Reduces pre-treatment dropout.
• Builds early momentum and trust.
2. Robust Early Services
BrightCare increases session frequency early in treatment to strengthen the therapeutic relationship and accelerate progress.
• Traditional models offer only two hours per month.
• Early, frequent contact improves adherence.
• Clients experience faster success, which increases motivation.
3. Treatment Completion
BrightCare reduces dropout by ensuring clients experience meaningful progress early in treatment.
• Early wins increase commitment.
• Clients remain engaged through the full treatment episode.
• Perceived effectiveness improves overall outcomes.
Coordinated Whole-Person Care
BrightCare integrates therapy and primary care from the start to ensure that medical contributors to mental health are addressed immediately.
• Reduces fragmentation found in traditional siloed systems.
• Ensures comprehensive, efficient, and effective care.
• Supports long-term wellness through collaborative communication.
Measurement-Based Care Standards
Assessment Requirements
BrightCare uses structured, recurring assessments to ensure treatment remains effective and responsive.
• Treatment plans: every 90 days
• PHQ-9 & GAD-7: every 90 days for clients 13+
• PSC-17: every 90 days for clients 12 and under
• BrightFIT score: added to every encounter
Automated Assessment Delivery
• System automatically sends PHQ-9 and GAD-7 every 90 days.
• Delivered to client portals; can also be completed in session.
• Auto-sent assessments expire after 30 days to prevent duplicates.
BrightFIT: BrightCare’s Feedback-Informed Treatment System
BrightFIT is Brightways Counseling Group’s version of Feedback-Informed Treatment (FIT), designed to ensure strong therapeutic alignment and continuous improvement in care quality.
BrightFIT Structure
At the start of every visit, clients complete three brief 1–10 rating questions:
• My therapist understands me.
• We are working on things that are most important to me.
• My therapist’s communication style is working for me.
These questions reflect the core conditions necessary for effective therapy and providereal-time insight into the client’s experience. BrightFIT supports:
Early identification of barriers to progress
Strengthening of the therapeutic alliance
Continuous alignment with client-identified goals
A culture of transparency, responsiveness, and collaboration
BrightFIT is not an evaluation of the client or the therapist — it is a structural component of the BrightCare model that ensures treatment remains client-centered, effective, and responsive.
Why BrightCare Works
BrightCare succeeds because it delivers what traditional models do not:
• Rapid access that dramatically increases engagement
• Robust early engagement that accelerates progress
• Coordinated whole-person care that addresses medical and psychological needs
• Continuous measurement and real-time feedback through standardized assessments and BrightFIT
Clients attend more consistently, adhere more reliably, complete treatment more often, and achieve better outcomes. BrightCare is clinically effective, fiscally responsible, andaligned with the needs of modern healthcare systems.
